Job Summary

Under the general supervision of the Clinical Supervisor, the CSW III is a licensed clinician with three years of case management experience who works within the Citywide Roving Housing First Team. The Roving Team provides short term Case Management services to adults who live in Permanent Supportive Housing in San Francisco through a subsidy from the city. Clients are referred to our program when they become at risk of eviction and a return to homelessness due behavior resulting from a mental health diagnosis. We triage this eviction risk though building relationship with clients, collaborating with onsite providers, linking clients to community providers and medical care, referrals to legal supports, and education and support to the system of care working with our clients. The goal of our program is to support individuals in staying housed. The CSW III will carry a caseload of individuals who have complex co-occurring physical health, mental health and substance misuse disorders to support them in accessing housing and maintaining housing and link to appropriate outpatient services. The CSW III will provide individual and/or group treatment including case management services, psychotherapy, community events, intake, assessment, service planning, crisis intervention and 5150 assessments, and consultation and coordination of services among a client's treatment team to help maximize housing retention and promote stability. The CSW III should possess a basic understanding of the DSM V mental health diagnostic criteria, Harm Reduction principles, Trauma Informed Care and Anti-Racism. Working with diverse and multicultural team and community, the incumbent will participate and contribute to culturally focused clinical interventions and will practice a commitment to cultural humility and anti-racism. The CSW III will work closely with team members and community partners to develop successful treatment strategies and provide advocacy and coordination with other agencies and families. The CSW III will record services in the client's electronic medical record and will coordinate closely with hospitals and clinic medical providers as well as other duties as assigned.

The CSW III for the Housing First contract has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities that support the Program Director and Clinical Supervisors including; provision of clinical trainings to staff in the weekly staff meeting, responding to urgent referrals, needs, and coverage across the team, creating and leading support/resource groups in the community setting, and other tasks as needed.

The job will involve the ability to lift 50 lbs., walking and climb stairs.

Department Description

Citywide Case Management Programs is a Division of the UCSF/ZSFG Department of Psychiatry, located at 982 Mission St. Its mission is to provide behavioral health intensive case management services to adults who suffer from serious mental illness, co-occurring substance abuse disorder and chronic medical conditions, experience significant systemic determinant of health, including justice-involvement or other forms of institutionalization, poverty and housing instability.

Required Qualifications

* Master's degree from an accredited school of social work, possession of a clinical social worker license issued by the California Board of Behavioral Science Examiners, and three years of post-master's experience; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position;
* Three years of Case Management experience
* Experience working with severely mentally ill adults, the homeless and dual diagnosed populations
* Experience doing community-based case management and the ability to do outreach.
* Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology
* Excellent communication and interpersonal skills
* Able to work as a team member in a culturally diverse setting
* Able to lift 50lbs and climb stairs
* Demonstrated excellent attendance and reliability
* A preliminary job offer and assignment may be extended pending successful background clearance.
* An understanding of mental health issues and sensitivity toward mental health clients
* Ability to work with clients and program staff from diverse cultural and social class backgrounds

Preferred Qualifications

* Demonstrated ability to work within multidisciplinary team to support clients
* Experience with Avatar Billing System
* Prior related experience in the homelessness response system of SF
* Demonstrated understanding of Harm Reduction, The Recovery Model and Housing First Model

License/Certification

  • CSW III: Current California State License of Clinical Social Work, Marriage and Family Therapists, or Licensed Professional Clinical Counselor in good standing.
