The Senior Manager/Regional Lead for the Office of Health Equity and Community Impact at Yale New Haven Health System will lead and manage staff, projects, and project teams to develop, advance, and execute on the Yale New Haven Health System core health equity and community impact work. With a focus on a specific Delivery Network (DN) region (Lawrence + Memorial/Westerly, or Greenwich/Bridgeport) the individual in this community facing role serves as a subject matter expert who will oversee the day-to-day operations of community engagement and impact work including community programs and projects, collective impact partnerships, the Community Health Needs Assessment (CHNA) development, and implementation and community program measurement and outcomes. With a focus on centering community voice and health equity in our work, strategies, and plans, he/she/they are responsible for establishing and enhancing community partnerships and relationships, overseeing our triennial community health needs assessment (CHNA) and community health improvement plan (CHIP) development, and are tasked with the responsibility for developing, supporting and /or implementing strategies and plans for collaboratively improving the health of our communities in measurable ways.

Responsibilities

  • 1. With the Executive Director, assess the state of community clinical partnerships across the health system
    • 1.1 Oversee the (CHNA) process with timely deliverables for the assigned region working collaboratively with the Executive Director, consultant/vendor, delivery network staff and community partners.
  • 2. Health Equity and Community Health Strategy Development and Implementation
    • 2.1 Works closely with the Executive Director to identify emerging health equity and diversity issues, create strategy to develop initiatives to guide the health system's goal of reducing health disparities and improving health equity for the communities it serves.
  • 3. Staff and Project management
    • 3.1 Develops, leads and manages health equity and community health staff including conducting performance reviews, professional development plans, performance enhancement plans when need, employee relations concerns etc.
  • 4. External facing community-based meetings and coalitions
    • 4.1 Represent YNHHS in coalition meetings, trainings, conferences, and other events.

Qualifications

EDUCATION

Bachelor's degree is required. Master's degree strongly preferred (business, healthcare administration, public health etc.) Demonstrated continuing education in the fields of community health and health equity preferred.

EXPERIENCE

Five years of public health, population health, patient care, healthcare leadership or related experience. Two years? experience supervising staff, preferably in a professional multi-disciplinary team setting. Experience working in a matrixed environment across multiple departments to achieve program outcomes. Experience working with project/program implementation, timelines, workplans etc. Demonstrated ability to establish effective interpersonal relationships with a broad range of internal and external constituents. Well-developed presentation and written skills including the ability to interact with a broad range of constituents including hospital executive leadership teams, board members, media, and the general public. Experience with grant writing preferred. Experience with healthcare research preferred. Basic understanding of public health statistics and data analysis.

Possesses a strong social justice perspective and demonstrated understanding of health equity and the structural drivers of health that impact marginalized communities. Possesses strong professional management and oral and written communication skills. Ability to work collaboratively with diverse community and other stakeholder groups. Demonstrated ability to foster cooperation, communication and consensus among groups and teams as well as lead, influence, coach, and guide others to help them achieve desired results. Demonstrated ability to collaborate and lead projects across teams.

PHYSICAL DEMAND

Position will require day and evening community meetings within the hospital and at community locations.
